Lighting, makeup, hair, and most importantly, photoshop all play a combined role in creating a look that just can't be attained by the average woman (did her neck really need to be longer??).
Searching 'photoshop makeovers' on YouTube yields so many examples of what exactly photoshop does and how it's used, like this video where they changed everything about this girl's looks using digital means only.
